a. Accomplished Application Form and Checklist
b. 2 valid (not expired) IDs
      Acceptable primary IDs are:
              1. Social Security System (SSS) / Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
              2. Land Transportation Office (LTO) Driver’s License.
              3. Professional Regulatory Commission (PRC) ID
              4. Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) card
              5. Commission on Elections (COMELEC) Voter's ID or Voter's Certification from the Election Officer with Dry Seal
              6. Philippine National Police (PNP) Firearms License
              7. Senior Citizen ID
              8. Airman License (issued August 2016 onwards)
              9. Philippine Postal ID (issued November 2016 onwards)
c. Proof of Billing
        We only accept the following Proof of Billing:
              e.g Utility bills, Home owners association fee billing or lease contract.
d. Payment of Installation Fee

You can apply in 2 ways:
- You may visit your building administrator and ask for the Application form and checklist.
- You may also go to application.weavetech.com.ph to apply online.

- Depending on the location and installation set up, our standard installation fee is Php 1,500 or Php 2,500.

- Our turnaround time is 5-7 working days.

- For serviceability checking, please fill up our online application form at application.weavetech.com.ph
- Serviceable buildings and areas are indicated within the application form.

- Installation Fee and Security Deposit equivalent to 1 monthly service fee, if applicable.
